What to Bring to Your Appointment
Please bring the following with you on the day of your visit:
Valid photo ID
Dental insurance card (if applicable)
Completed patient forms (if not submitted online)
List of current medications and any relevant medical history
If you’re receiving a specialized service such as Invisalign, dental implants, or tooth extractions, feel free to ask our team for any additional pre-appointment instructions.
